                                    4. To be provided with room for family to spend the night.5. To receive visitors, including children, at any hour.AS A PATIENT YOU HAVE THE RESPONSIBILITY TO:1. Provide Samaritan with accurate and complete health, insurance, and/or financial information.2. Notify Samaritan of changes in your physician or insurance coverage.3. Remain under licensed physician care while receiving services from Samaritan.4. Supply medication, equipment, or supplies that Samaritan does not provide.5. Notify Samaritan if you wish to cancel service or treatments prescribed, and subsequently accept the consequences of choices made.6. Participate in developing your plan of care and updating it as your condition changes.7. Treat Samaritan personnel with dignity, courtesy, and respect.8. Inform Samaritan personnel of any complications or side effects of prescribed activities, medications, etc. in your plan of care.9. Notify Samaritan of the existence of, or changes to, advance directives and cooperate with Samaritan in meeting the requirements of an advance directive, if one is executed.10.Communicate changes in the plan of care to Samaritan personnel and follow through on the established emergency plan.11.Notify Samaritan if a fall and/or injury occurs or if a medication error occurs.12.Comply with medications and prescribed treatment as directed by the physician.13.Maintain a safe environment.14.Notify Samaritan if you are unavailable for a scheduled visit.15.Ask questions about your care and/or any aspect of Samaritan services.16.Meet any accepted financial commitment to Samaritan.17.Follow rules and regulations as identified by Samaritan.54 Programs and Policies
