50 Programs and PoliciesUNDERSTANDING MEDICARE-DETERMINED DISCHARGES FROM HOSPICE PROGRAMA. Revocation: You may choose to withdraw from the hospice program at any time. If you are utilizing the Medicare Hospice Benefit, you must sign a Statement of Revocation. The remaining days of the current benefit period in which you are revoking are forfeited. Remaining benefit period(s) are available for future use.B. Transfer: You may transfer to another Medicare-certified hospice program once in every benefit period.C. Reasons for Hospice-initiated discharge:%u2022 You move away from Samaritan%u2019s service area or go to a facility not contracted with Samaritan.%u2022 Your condition stabilizes or improves and you are no longer considered terminally ill.%u2022 You are discharged for cause: Violence or threatened violence against Samaritan staff or the patient/family unit, or for a continued unsafe environment.D.
