SamaritanNJ.org 49UNDERSTANDING LEVELS OF HOSPICE CARE%u2022 Routine level of hospice care is when hospice care is provided in your home, assisted living or nursing home. The majority of hospice care is provided at home where you or your loved one can be in a familiar setting surrounded by family.%u2022 Respite care is provided %u2013 only when necessary and on an occasional basis %u2013 to give family members or other caregivers rest. Respite can only be provided at a Medicare-certified inpatient hospice facility, or a Medicare-certified skilled nursing facility that has the capacity and ability to provide around-the-clock nursing care. Respite care, however, cannot be provided in your home.%u2022 Continuous home care is similar to the general inpatient level of hospice care except that it is provided in your home during periods of crisis or when symptoms are harder to control and sustained nursing care is required.%u2022 General inpatient level of hospice care is recommended by your hospice nurse when severe pain or other symptoms require an advanced level of care that is more safely and effectively provided during a short stay in an inpatient hospice facility.UNDERSTANDING INPATIENT HOSPICE CAREYour hospice care team will do their best to manage symptoms in your home environment. These symptoms include pain, trouble breathing, nausea and vomiting, and severe anxiety or restlessness. Your hospice team will work with you, your family, and your physician to make any necessary changes to you or your loved one%u2019s medicines or other treatments to relieve these symptoms.However, sometimes these symptoms do not respond to the regular medications we have available in the home environment. When these symptoms do not improve over 24 to 48 hours, then our most effective plan is to transfer you, or your loved one, to one of our inpatient centers so we can manage these symptoms more quickly. We recommend this approach because nurses and physicians are available 24/7 to make any needed adjustments in a peaceful, home-like environment. In our experience, this is the best choice to keep your loved one, and you, as comfortable as possible. Your home hospice team will work with our inpatient teams to make sure we accomplish your goals of care.Additionally, some patients may be admitted directly to one of our inpatient centers depending upon their circumstance.Please reach out at any time to your nurse or social worker with any questions.
