If this information would be detrimental to your health, or if you are not able to understand the information, the explanation shall be provided to a family member or an individual who is a legal representative of the patient and documented in the patient%u2019s medical record.15.Receive, as soon as possible, the services of a translator or interpreter to facilitate communication between you and health care personnel.16.Refuse services, including medication and treatment, provided by the facility and to be informed of available hospice treatment options, including the option of no treatment, and of the possible benefits and risks of each option.17.Refuse to be interviewed/photographed/videotaped without your written consent and without the supervision of a member of the community relations department.18.Refuse to participate in experimental research. If you choose to participate, then written informed consent shall be obtained.19.Be free from mistreatment, neglect, verbal, and physical abuse including injury of unknown source. Be free of exploitation and misappropriation of patient property. Be free from restraints, unless they are authorized by a physician for a limited period of time to protect the patient or others from injury.20.Be assured of confidential treatment of your medical health record, and to approve or refuse in writing its release to any individual outside the hospice, except as required by law or third party payment contract.21.Retain and exercise to the fullest extent possible all the constitutional, civil, and legal rights to which you are entitled by law, including religious liberties, the right to independent personal decisions, and the right to provide instructions and directions for health care in the event of future decision making incapacity in accordance with the New Jersey Advance Directive for Health Care Act, N.J.S.A. 26:2H-53 et seq., and any rules which may be promulgated pursuant thereto.22.Discharge yourself from treatment by the hospice or be transferred to another hospice provider.23.Be protected from neglect, exploitation, and abuse.24.Have the organization provide accommodations for family privacy after patient death.25.Space for private family visiting.26.Have the organization provide room for family to spend the night.27.Receive visitors and children at any hour.AS A PATIENT RECEIVING HOSPICE CARE AT ONE OF SAMARITAN%u2019S INPATIENT CENTERS, YOU HAVE THE FOLLOWING ADDITIONAL RIGHTS:1. To be protected from neglect, exploitation, and abuse.2. To be provided with accommodations for family privacy after the death of the patient.3.
