48 Programs and Policies%u2022 Samaritan is supported by the United Way, memorial contributions (donations made in memory of, or in honor of a loved one), individual and corporate donations, and grants.** If you call 911 or go to the hospital without notifying Samaritan, or the hospital visit is not in your hospice plan of care, you may be responsible for the bill. **ELIGIBILITYFOR MEDICARE BENEFICIARIES %u2013 MEDICARE PART A.%u2022 Prognosis of less than six months, certified by your primary physician and a hospice physician.%u2022 You have a serious and progressive illness.%u2022 You no longer wish to receive aggressive treatment to cure your disease or to prolong your life.%u2022 You and your and family understand, agree with, and accept the hospice concept and method of care.%u2022 Your primary physician approves entry into hospice care, endorses the hospice concept, and participates in the hospice method of care.%u2022 If your loved one is unable to speak for themselves, they may be admitted to the hospice program if there is a legal guardian to sign for them, a durable power of attorney for medical purposes, or other evidence that they desire comfort services as opposed to aggressive treatment in the end stage of their disease.LIVING ALONESamaritan is able to accept patients who live alone. Your social worker will work with you to assist with additional needs like lock boxes, lifelines, and private-pay home health aide hours. Your social worker will also work with you to develop a secondary plan should you become unable to care for yourself. This plan may include nursing home placement, private pay live-in help, relocation to a family member%u2019s home, etc.HOSPICE IN A NURSING HOME OR ASSISTED LIVINGSamaritan has contracts with many area facilities to provide hospice services. If a patient or family requests hospice services in a facility within Samaritan%u2019s service area and there is no previously established contract, Samaritan will try to negotiate a contract.
