                                    30 Safety and SuppliesMedication, Supplies, and EquipmentPHARMACY SERVICESWHY AM I USING A HOSPICE PHARMACY?Your hospice benefits include full coverage of all medications on the approved medications list that are related to your hospice diagnosis. A hospice pharmacist is part of your hospice care team and works with your doctor and hospice nurses to develop a plan of care to meet your individual needs.HOW WILL THE HOSPICE PHARMACY KNOW WHEN I NEED NEW MEDICATIONS OR REFILLS?Your hospice nurse will call the hospice pharmacy to order and ensure you receive the medications you need%u2014whether those are new or refills. Your caregiver may pick up your medicine at your local pharmacy. Otherwise, your medications will be delivered to you by way of a mail courier in an unmarked package. You should get medicines not hospice covered from your regular pharmacy.WILL SAMARITAN PAY FOR ALL MY MEDICINES?Samaritan will pay for all medicines that are related to your primary hospice diagnosis and that are included in the approved medication list. Your hospice nurse will review these with you and your caregiver at the time of admission and as your needs change.Some medications may not be covered by hospice or by your Medicare D plan. Your hospice nurse will review the medication list that will be covered by hospice, those that will be covered by Medicare Part D plan, and those which may be your responsibility (if any).MEDICATIONUsage%u2022 Medications, both those prescribed by a health care provider and those you can get without a prescription, may be used to help relieve the pain or symptoms you experience because of your illness. %u2022 Your hospice nurse will routinely monitor your medications to be sure they are effective for treating the pain and/or symptom for which they were intended. %u2022 He/she will also ask you about any side effects you may be experiencing or any adverse reactions that might have occurred after taking a medication. %u2022 The hospice nurse keeps your doctor and the other members of your hospice care team informed of your medications and their effectiveness.Side Effects or Adverse Reactions%u2022 Each medication provided by Samaritan through the pharmacy provider will be delivered with a written instruction sheet that includes information regarding possible side effects or
