We provide compassionate care to thousands of seriously ill patients and their families every year, and understand that no two patients or families are alike. Your values and wishes matter, and your hospice nurse, physician, and other team members will work with you to design a plan of care to meet your unique physical, spiritual, and emotional needs.DURATION AND NUMBER OF VISITSThis individualized hospice plan of care will also reflect the frequency and duration of visits by hospice team members. Throughout the course of your hospice experience, there may be times when you require more or less visits by your hospice nurse, social worker, spiritual support counselor, certified home health aide, volunteer, or other team members. Your interdisciplinary team routinely reviews your needs at regular interdisciplinary team meetings. All information discussed remains confidential. Any changes are reviewed with you to ensure that your needs are met. If you would like to attend or participate in a team meeting, please notify your nurse.HOSPICE PHYSICIAN | NURSE PRACTITIONERYour hospice provider approves your admission to Samaritan%u2019s hospice program. They participate in team meetings to review the care you are receiving and offer advice and information. Your hospice physician serves as a communication link between Samaritan and physicians in the community and recertifies you for continued hospice care.YOUR ATTENDING PHYSICIANYour attending physician refers you to Samaritan. They oversee your hospice plan of care and are responsible for initial and follow-up orders for care. Your attending physician also works closely with your hospice nurse.REGISTERED NURSE (RN) | PRIMARY NURSEYour nurse visits you regularly. The nurse coordinates all your hospice services and provides skilled care including symptom management, pain control, teaching, and support. They work with your physician and family to anticipate needs. The hospice nurse supervises the LPN and CHHA.LICENSED PRACTICAL NURSE (LPN)Your licensed practical nurse provides skilled nursing care and assists with ordering supplies and meds, monitoring symptoms and vital signs, and wound care.CERTIFIED HOME HEALTH AIDE (CHHA)The certified home health aide assists with personal care, meal preparation, and other activities of daily living such as bathing and dressing.Your Hospice Team of Professionals
